Overview

The Care Coordinator will support Maximus patients by delivering a seamless care experience, managing provider schedules, and overseeing patient communications.

In Short

• Coordinate video conferencing schedules for provider consultations.
• Liaise with lab and pharmacy partners for test orders and prescriptions.
• Manage patient messaging queues and triage inquiries.
• Provide support and education to patients regarding telehealth services.
• Maintain electronic health records (EHR) with data integrity.
• Ensure compliance with healthcare regulations.

Requirements

• 1–3 years of experience in care coordination or telehealth support.
• Associate’s or Bachelor’s degree in Healthcare Administration or related field preferred.
• Familiarity with EHR systems and telehealth platforms.
• Excellent verbal and written communication skills.
• Strong attention to detail and organizational skills.
• Empathetic and patient-focused approach.
• Understanding of HIPAA and data privacy guidelines.
