Overview

The Care Coordinator provides comprehensive and person-centered care coordination and management services for members experiencing emotional and/or behavioral problems or complex chronic conditions. This role involves working with members, their families, and a network of provider agencies to improve healthcare access and outcomes.

In Short

  • Conduct strength-based assessments for members.
  • Assist in transitioning members to appropriate levels of care.
  • Deliver high-intensity care management services.
  • Facilitate access to community services.
  • Develop individualized service plans in collaboration with members.
  • Provide wraparound services tailored to member needs.
  • Follow-up with members to ensure continuity of care.
  • Maintain accurate records in the EMR system.
  • Collaborate with care team members and facilitate meetings.
  • Participate in on-call rotation.

Requirements

  • Associate’s Degree in psychology, sociology, or related field required.
  • Excellent organizational and office skills.
  • Strong people skills and ability to work with diverse populations.
  • Effective oral and written communication skills.
  • Detail-oriented with excellent time-management skills.
  • Ability to handle change productively.
  • Valid NYS driver’s license required.
  • Completion of pre-employment health requirements.
  • Mandated Reporter Certification required post-hire.
  • Experience in care coordination preferred.
