Overview

The Care Coordinator will provide assessments and case management services to clients with health disabilities at risk of nursing home placement, ensuring their overall well-being and independent functioning.

Requirements

  • Bachelor’s degree in social work or related field.
  • Licensed Practical Nurse (LPN) with current license acceptable.
  • Two years’ experience in human services or health-related field.
  • Excellent communication and organizational skills.
  • Proficiency in computer skills.
