Overview

The Care Coordinator is responsible for assessing and evaluating the needs of members to help them achieve optimal wellness by guiding them towards appropriate resources.

Requirements

  • Bachelor's degree in social sciences or related field, or relevant nursing/counseling licenses.
  • 1+ years of experience in healthcare and case management.
  • Experience with Medicare and Medicaid recipients.
  • Intermediate computer skills, especially in Microsoft Office.
  • Exceptional communication and interpersonal skills.
  • Valid driver's license and insurance required.
