Care Coordinator - Remote

Overview

We are seeking an extremely organized, dedicated, kind, patient, and compassionate Care Coordinator to join our team and play a vital role in ensuring our members receive seamless and efficient care through their childbirth journey.

In Short

• Conduct phone, chat and/or video intakes with members.
• Match families with suitable Doulas based on individual needs.
• Provide ongoing support and resources to families throughout their pregnancy.
• Understand and assist with Pacify technology for scheduling.
• Ensure completeness and compliance of client chart files.
• Communicate effectively with the Pacify team for coordinated care.
• Maintain accurate member records in EHR systems.
• Strong organizational and time management skills required.
• Compassionate and empathetic with a desire to help others.
• Spanish speaker preferred.

Requirements

• High school diploma or equivalent required.
• 1-2 years of experience in a member-facing role in healthcare.
• Excellent communication and interpersonal skills.
• Strong organizational and time management skills.
• Understanding of basic medical terminology preferred.
• Ability to work independently and as part of a team.
• Highly detail-oriented and accurate.
