Care Coordinator - Remote

Overview

As a Care Coordinator, you will work closely with our providers and patients through many stages of their Thirty Madison journey. The role is both patient and provider-facing, from helping patients receive medical records, to assisting providers in coordinating treatment plans. You’ll partner with and support the Customer Experience team, our medical network, and patients across all of our brands. A successful Care Coordinator must be comfortable speaking with others across various channels (primarily email/message) compassionately and professionally. Patients are ready for healthcare delivery that matches the innovation and accessibility they’ve come to expect from other service sectors, and Thirty Madison is putting that capability in their hands. In Short

• Work collaboratively with a multi-disciplinary team to ensure a high-quality patient experience.
• Ensure processes and procedures are being followed for effective care delivery.
• Identify and escalate potential quality issues in care.
• Manage patient/provider messaging and order fulfillment queues.
• Provide administrative support for the provider network.
• Coordinate treatment and community resources for patients.
• Other duties as assigned.

Requirements

• One year of experience in a medical practice.
• Excellent verbal and written communication skills.
• Attention to detail in patient care.
• Service-oriented mindset with empathy for patients.
• Self-motivated and willing to help navigate the healthcare industry.
• Skilled in using online tools and technology.
• Reliable attendance and adherence to work schedule.
