Overview

The Unite Us Care Coordinator plays a pivotal role in supporting customers who have purchased social care coordination services by leveraging the Unite Us Platform to ensure the delivery of high-quality care coordination services. This position entails interacting directly with clients seeking services and collaborating with partner organizations to facilitate resource allocation and support for clients. Care Coordinators are empathic individuals who serve as platform experts, proficient in conducting client needs assessments, guiding clients in accessing social determinants of health resources, and closing gaps in care.

In Short

• Manage a caseload of customers' clients that averages 50 referrals completed weekly.
• Utilize the Unite Us Platform to facilitate seamless care coordination services.
• Conduct intake and needs assessment screenings for clients.
• Collaborate with partner organizations to identify and connect clients with appropriate services.
• Serve as a liaison between clients and service providers.
• Conduct follow-up with clients and service providers.
• Provide supportive feedback to community-based organizations.
• Continuously expand knowledge of community resources.
• Deliver excellence in customer experience.
• May support special projects and backend workflows.

Requirements

• Minimum of one year of experience in navigating social services.
• A degree in social work or a relevant field preferred.
• Clinical credentials preferred.
• Empathy and passion for helping vulnerable populations.
• Quick learner excited to become an expert in new technologies.
