Humana Gold Plus Integrated is seeking Care Coordinators to assess and evaluate member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families towards and facilitating interaction with resources appropriate for the care and wellbeing of members.

In Short

• Care Coordinators assess and evaluate member needs.

• Requires local travel (75-90%) to visit members.

Requirements

• Bachelor’s degree in social sciences or related field, or relevant licensure.
• 1 year of experience in health care or case management.
• Experience with Medicare & Medicaid recipients.
• Intermediate to advanced computer skills.
• Exceptional communication and interpersonal skills.
• Ability to manage member care effectively.
• Must pass TB screening.
• Must have a dedicated workspace for remote work.
• Ability to work independently and manage time effectively.
• Valid driver’s license and vehicle insurance required.
