In Short

• Conduct telephonic outreach to enroll patients into care programs.
• Collaborate with Nurse Care Managers to develop personalized care plans.
• Coordinate care transitions across healthcare settings.
• Assess and address Social Determinants of Health.
• Navigate patients through health systems and community resources.
• Advocate for patients and ensure timely interventions.
• Maintain clear communication with patients and clinical teams.
• Document all interactions in compliance with regulations.
• Educate patients and their families on health management.
