Overview

The Care Coordinator role focuses on providing exceptional customer service and support to members navigating their health and care journey.

In Short

  • Put people first, making decisions with the member’s best interests in mind.
  • Communicate and follow-up with plan members to ensure understanding of the program.
  • Facilitate patients through the surgery review process.
  • Manage a case load across a variety of clients and surgical categories.
  • Coordinate the scheduling and approval process for surgeries.
  • Obtain necessary information for the referral process.
  • Collaborate with providers and clinics to resolve issues.
  • Maintain confidentiality and comply with HIPAA regulations.
  • Demonstrate Transcarent core values.
  • Be willing to work overtime as needed.

Requirements

  • 2 to 4 years of customer relationship and phone center experience.
  • Proficient in data entry and Microsoft Office.
  • Excellent customer service and communication skills.
  • Prior healthcare industry experience preferred.
  • Strong organizational and problem-solving skills.
  • Ability to manage a high-volume caseload.
  • Strong attention to detail.
  • Ability to work in a dynamic environment.
  • Experience in surgical scheduling is a plus.
  • Willingness to adapt to changes positively.
