In Short

• Fully remote position working in the Eastern time zone.
• Conduct monthly clinical check-ins with patients.
• Identify and address clinical concerns and reinforce care plans.
• Ensure clinical documentation accuracy across EHR and partner technology.
• Collaborate closely with providers and care teams.
• Utilize SMS and phone outreach for patient engagement.
• Maintain high call quality and adherence to workflows.
• Participate in continuous education and quality improvement.
• Document all patient interactions clearly for compliance.
• Support adherence to medications and lifestyle changes.

Requirements

• Active LPN or LVN license.
• 1–2 years of clinical experience in ambulatory care or population health.
• Strong clinical assessment and communication skills.
• Ability to manage a structured workflow.
• Intermediate skills with MS Office Suite.
• Able to work effectively in a remote environment.
• Home internet must support minimum speed requirements.
• Dedicated workspace for protecting patient information.
• Experience in chronic disease management preferred.
• Familiarity with telehealth programs is a plus.
