Overview

The Transition of Care Coach works with members, providers, and multidisciplinary teams to assess, facilitate, plan, and coordinate integrated delivery of care for patients with high needs, ensuring quality and cost-effective outcomes.

In Short

  • Remote position with field travel in Thurston or Mason County.
  • Requires WA state LICSW, LMFT, or LMHC licensure.
  • Experience in inpatient settings and Medicaid preferred.
  • Conducts face-to-face visits and home visits for high-risk members.
  • Coordinates care using the Coleman Care Transitions Model.
  • Educates members on health management and care transitions.
  • Utilizes motivational interviewing techniques.
  • Facilitates interdisciplinary care team meetings.
  • 1-3 years of relevant experience required.
  • Bilingual candidates encouraged to apply.

Requirements

  • Completion of an accredited LVN or LPN program, or a degree in social science or related field.
  • 1-3 years in case management or behavioral health settings.
  • Active, unrestricted state license if applicable.
  • Valid driver's license and reliable transportation.
  • Preferred experience of 3-5 years in similar roles.
