Overview

The Care Manager—Registered Nurse is a key member of our Special Needs Plan (SNP) care team, responsible for coordinating care for members who often face multiple chronic medical and behavioral health conditions, as well as various social determinants of health (SDoH) needs.

In Short

  • Conduct comprehensive assessments to evaluate members’ needs.
  • Address SDoH challenges by connecting members with resources.
  • Develop and implement individualized care plans.
  • Monitor member progress and advocate for necessary services.
  • Engage with members telephonically 50-75% of the day.
  • Provide disease management education and support.
  • Ensure documentation of care management activities.
  • Collaborate with the interdisciplinary care team.
  • Meet regulatory requirements within specified timelines.
  • Support team objectives and enhance operational efficiency.

Requirements

  • Active and unrestricted Registered Nurse (RN) licensure in NY.
  • Minimum 3+ years of nursing experience.
  • Minimum 2+ years of case management experience.
  • Proficient in Microsoft Office Suite.
  • Access to a private space for work.
  • Confidence working independently from home.
  • Experience with Medicare and Medicaid care management preferred.
  • Familiarity with community resources.
  • Excellent communication skills.
  • Bilingual skills, especially English-Spanish preferred.
