Overview

The Care Manager—Registered Nurse is responsible for coordinating care for members with chronic medical and behavioral health conditions, providing education, developing care plans, and ensuring optimal health outcomes.

Requirements

  • Active Registered Nurse (RN) license in Michigan.
  • 3+ years of nursing experience.
  • 2+ years of case management experience.
  • Proficient in Microsoft Office Suite.
  • Ability to work independently from home.
  • Experience with Medicare and/or Medicaid care management preferred.
  • Bilingual skills (English-Spanish) preferred.
  • Associate’s degree in Nursing required; BSN preferred.
