Case Manager, Registered Nurse - Remote

Overview

The Care Manager—Registered Nurse is responsible for coordinating care for members with chronic medical and behavioral health conditions, connecting them with resources and support services.

Requirements

• Active RN licensure in Michigan.
• 3+ years of nursing experience.
• 2+ years of case management experience.
• Proficient in Microsoft Office Suite.
• Ability to work independently from home.
• Familiarity with healthcare technology tools.
• Excellent communication skills.
• Experience with Medicare/Medicaid members preferred.
• Bilingual skills are a plus.
• ASN degree required; BSN preferred.
