Overview

The Care Manager—Registered Nurse is a vital member of the Special Needs Plan care team, responsible for coordinating care for members with chronic medical and behavioral health conditions, addressing social determinants of health needs.

Requirements

• Active RN licensure in Ohio or compact licensure.
• Proficient in Microsoft Office Suite.
• 3+ years of nursing experience.
• 2+ years of case management or home healthcare experience.
• Experience with Medicare and Medicaid care management preferred.
• Ability to work independently and effectively from home.
• Strong communication and customer service skills.
• Experience with health-related assessments.
• Bilingual skills are a plus.
• ASN degree required; BSN preferred.
