Transition of Care Associate, Licensed Practical Nurse - Remote

Overview

As a Transition of Care Associate, you will play a crucial role in coordinating care for members experiencing significant health changes, ensuring a seamless transition to the next care setting and facilitating their post-discharge goals.

In Short

• Full-time telework position with hours Monday to Friday, 8:00 AM to 5:00 PM EST.
• Responsible for care coordination for members post-discharge.
• Ensure members understand their medications and benefits.
• Monitor members for alerts or changes in condition.
• Facilitate interdisciplinary care team meetings.
• Complete outreach cadence calls and post-discharge questionnaires.
• Utilize reporting to reduce emergency department utilization.
• Engage with community resources to support members.
• Must have 2 years of LPN experience and active NJ licensure.
• Bilingual skills preferred.

Requirements

• 2 years of Licensed Practical Nurse (LPN) nursing experience.
• Active and unrestricted LPN licensure in NJ or compact licensure.
• 1 year of advanced proficiency in Microsoft Word, Excel, and Outlook.
• Discharge planning experience.
• Effective verbal and written communication skills.
• High School Diploma or equivalent GED.
• Associate's Degree or Bachelor's Degree preferred.
• Ability to multitask and adapt to a fast-paced environment.
• Bilingual (English/Spanish) is a plus.
• Must be an LPN.
