Overview

As an RN Care Manager at Strive Health, you will play a pivotal role in transforming the kidney care system by providing specialized clinical support to patients with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). Your work will involve ensuring safe transitions between care settings and improving patient outcomes through comprehensive care coordination.

In Short

  • Serve as a clinical resource for patients with CKD and ESRD.
  • Support safe and effective transitions between care settings.
  • Work closely with interdisciplinary care teams.
  • Ensure compliance with NCQA accreditation standards.
  • Conduct virtual assessments and proactive outreach.
  • Manage a diverse patient panel and craft personalized care plans.
  • Communicate updates to care teams and families.
  • Utilize data platforms to identify trends and areas of focus.
  • Participate in quality improvement initiatives.
  • Provide culturally sensitive care to diverse patients.

Requirements

  • Associate’s degree from an accredited school of nursing.
  • Current Registered Nurse (RN) license in the state of practice.
  • 3+ years of related education, experience, or certification.
  • Current BLS or CPR Certification required.
  • Efficient and reliable transportation for patient visits.
  • Proficiency in Microsoft 365 Suite.
  • Experience in nephrology, ER, ICU, or complex case management preferred.
  • Certified Case Manager (CCM) certification preferred.
  • Experience with electronic medical records (EMR).
  • Strong communication and analytical skills.
