Overview

The RN Case Manager for the Transitions of Care program will coordinate with hospital case managers to assist members in their post-discharge journey, ensuring they receive the necessary support and resources to avoid readmission.

• Coordinate with hospital case managers for member assessments.
• Conduct in-home or virtual discharge visits.

• Participate in inpatient rounds and post-discharge case conferences.

• Perform regular check-ins and follow-ups with members.
• Conduct medication reconciliation and quality gap closure.
• Document member interactions and track care metrics.
• Meet members in various community settings.
• Support both clinical and non-clinical needs of members.

Requirements

• 3+ years of nursing experience.
• Graduate of an accredited nursing program (R.N.).
• Experience with care management and transitions of care.
• Understanding of behavioral health issues and tools.
• Strong communication and collaboration skills.
• Ability to work in community settings.
• Proficient in electronic health records.
• Knowledge of ICD and CPT coding.
