RN - Care Manager Transition of Care - Remote

Overview

This remote role serves members transitioning from inpatient hospitalizations, performing care management duties to assess, plan, and coordinate medical and supporting services for post-discharge members.

In Short

• Perform care management duties for post-discharge members.
• Complete medication review for pre-admission and post-discharge reconciliation.
• Develop care/service plans in collaboration with healthcare teams.
• Facilitate the transition into active care management based on member needs.
• Assess member health status and resource needs.
• Conduct outreach to assess member needs and collaborate with resources.
• Document and maintain member information for compliance.
• Provide feedback to leadership on care and quality delivery improvement.
• Comply with all policies and standards.
• May perform other duties as assigned.

Requirements

• Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing.
• 2 – 4 years of related experience.
• Active Illinois RN credential preferred.
• Experience in service coordination, case management, and discharge planning.
• Licensure such as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required.
