In Short

  • Work closely with the care team to craft personalized care plans.
  • Conduct regular assessments onsite and virtually.
  • Initiate proactive outreach to patients and clinics.
  • Manage a diverse patient panel adhering to NCQA standards.
  • Communicate updates to the care team and family members.
  • Act as a liaison with dialysis facilities.
  • Leverage data platforms to identify trends and areas of focus.
  • Provide culturally sensitive care to meet diverse patient needs.
  • Utilize clinical judgment and analytical skills in care delivery.
  • Maintain accurate program software records.

Requirements

  • Associate’s degree from an accredited school of nursing.
  • Current Registered Nurse (RN) license in the state of practice.
  • 3+ years of related education, experience, or certification.
  • Current BLS or CPR Certification required.
  • Efficient and reliable transportation for patient visits.
  • Internet connectivity with specified minimum speeds.
