RN Case Manager - Hybrid/Remote

Overview

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support some of our most complex patients, assisting them in navigating a challenging healthcare system.

In Short

• Support complex patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD).
• Work autonomously and collaboratively with healthcare teams.
• Coordinate patient care and transitions.
• Manage ongoing patient needs and establish treatment plans.
• Identify and address social determinants of health.
• Conduct medical record reviews and consultations.
• Flexible Monday-Friday schedule with telehealth and travel requirements.
• Minimum of 5 years RN experience required.
• Current RN License, CPR or BLS certification required.
• Intermediate computer skills required.

Requirements

• Passion for delivering care to patients with complex, chronic illnesses.
• Current RN License required, BSN preferred.
• Minimum of 5 years RN experience required.
• Current CPR or BLS certification.
• Willingness to travel within an assigned territory.
• Proficiency in MS Word, Excel, and Outlook.
• Home office with internet connectivity required.
• Spanish-speaking preferred.
