RN Case Manager - Remote

Overview

The RN Case Manager coordinates all aspects of a patient’s care and serves as a patient advocate across the continuum of care in partnership with the patient’s providers and extended care team.

• Conduct initial intake calls and patient outreach.
• Provide ongoing care coordination for complex patients.
• Conduct needs assessments and develop care plans.
• Support patients during care transitions.

Requirements

• Current RN License in good standing.
• Bachelor's degree in nursing required.
• Minimum of 4 years clinical experience.
• Certified Case Manager (CCM) certification preferred.
• Strong knowledge of clinical best practices.
• Ability to manage multiple clients.
• Proficient in Microsoft Office and clinical systems.
• Excellent communication skills.
• Ability to work in a fast-paced environment.
• Demonstrated ability to perform multiple tasks.
