In Short

• Full-Time Hybrid opportunity based in Denver, CO.
• Responsible for overseeing comprehensive care and coordination of services for patients.
• Manage a diverse caseload ensuring quality patient care.
• Provide coaching and support to Certified Nursing Assistants (CNAs).
• Develop and implement individualized care plans.
• Maintain accurate and timely electronic charting.
• Establish effective communication with healthcare providers.
• Participate in on-call rotation and holiday coverage.
• Monitor patient readmission rates and caregiver satisfaction.

Requirements

• Associate's or Baccalaureate Degree in Nursing.
• Current Registered Nurse license in Colorado.
• 2 years of clinical experience; home health experience preferred.
• Experience in case management and pediatric/adult care preferred.
• Reliable transportation and valid driver's license required.
• Ability to work flexible hours and provide on-call service.
• Knowledge of OASIS preferred.
