The Patient Care Coordinator manages in-home and virtual appointments for providers and clinical staff, utilizing scheduling software to optimize schedules. This role ensures appointments align with patient needs, provider availability, and organizational goals.

Requirements

• High school diploma or GED.
• 2+ years work experience in a healthcare setting.
• Preferred: Call center or scheduling experience.
• Preferred: Experience with patient communication protocols.
• Preferred: Experience in a value-based healthcare setting.
