Our Patient Care Coordinators manage in-home and virtual appointments for providers and clinical staff, utilizing scheduling software to optimize schedules. This role ensures appointments align with patient needs, provider availability, and organizational goals.

• Coordinate and schedule appointments for providers and clinical staff, both in-person and virtual visits.

• Maintain accurate records of all scheduled visits, cancellations, and reschedules.
• Communicate with patients regarding appointment confirmations and rescheduling.
• Work with the interdisciplinary team to ensure a smooth patient experience.
• Support provider and clinical staff productivity targets by optimizing schedules.
• Provide timely reporting and updates on scheduling metrics.
• Comply with all relevant healthcare laws and regulations.

Requirements

• High school diploma or GED.
• 2+ years work experience in a healthcare setting.
• Preferred: Call center or scheduling experience.
• Preferred: Experience with patient communication protocols.
• Preferred: Experience in a value-based healthcare setting.
• Proficient in using scheduling software and EHR systems.
• Knowledge of HIPAA regulations preferred.
• Strong attention to detail and organizational skills.
• Effective problem-solving skills for handling scheduling conflicts.
• Excellent communication skills for coordinating with providers and patients.
