Patient Care Coordinator - Remote

Form Health is a national telemedicine weight loss clinic specialized in helping patients with obesity lose weight and lead healthier lives. This role is critical to our company mission of outstanding clinical care.

In Short

• Provide comprehensive support for daily clinical operations.
• Manage a panel of patients as their primary point of contact.
• Coordinate exceptional care and patient inquiries.
• Schedule and reschedule patient appointments.
• Ensure consistent patient engagement with the clinical team.
• Assist with prior authorization and prescription tasks.
• Provide administrative support for clinicians.
• Communicate patient feedback to improve operational tools.
• Contribute to special projects as directed.
• Work collaboratively with physicians and dietitians.

Requirements

• Associate’s or Bachelor’s degree preferred; CMA certification strongly preferred.
• Strong customer service drive and passion for helping patients.
• At least two years of experience in a medical practice.
• Excellent verbal and written communication skills.
• Highly organized with strong coordination and prioritization skills.
• Proficient in computer skills including Slack and Google Workspace.
