Overview

DaVita Integrated Kidney Care is seeking a Full-Time RN Case Manager to provide support to complex patients, helping them navigate the healthcare system through a hybrid remote model.

In Short

• Full-time RN Case Manager position.
• Hybrid remote work with in-person visits in Fresno, CA area.
• Assist patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD).
• Coordinate care and implement treatment plans.
• Identify social determinants of health affecting patients.
• Flexible Monday-Friday schedule.
• Requires current RN license and CPR/BLS certification.
• Minimum of 2 years RN experience required.
• Intermediate computer skills required.
• Spanish-speaking preferred in some markets.

Requirements

• Current RN License required, BSN preferred.
• Minimum of 2 years RN experience with healthcare performance coaching.
• Current CPR or BLS certification.
• Willingness to travel within an assigned territory.
• Home office with internet connectivity required.
• 2+ years of Case Management or Chronic Care Management preferred.
• 1+ years of utilization management experience preferred.
• Ability to build relationships with patients and families.
• Passion for delivering care to patients with complex illnesses.
• Flexibility to pivot quickly on priorities.
