Remote RN Case Manager

Overview

As a Remote RN Case Manager at Guidehealth, you will be responsible for partnering with clinical teams to provide complex case management, helping patients achieve optimal health outcomes.

• Comprehensively assess healthcare and psychosocial needs of patients.
• Develop and maintain individualized care plans.
• Provide evidence-based clinical interventions.
• Collaborate with patient navigators for non-clinical needs.
• Monitor patient data and adjust care plans accordingly.

• Maintain documentation in electronic medical records.

Requirements

• Licensed Registered Nurse in Georgia or Compact license.
• 3+ years of RN Case Management experience in outpatient settings.
• CCM certification preferred, must obtain within 1 year.
• Supervisory experience preferred.
• Excellent communication skills for patient interaction.
• Proficient in EMR and documentation systems.
• Ability to meet accreditation and quality standards.
