Overview

As an RN Case Manager, you will partner with clinical teams to provide complex case management and strengthen the connection between the patient, the primary care physician/medical practice staff, and the patient’s care team.

In Short

  • Conduct in-depth telephonic assessments to understand each patient’s needs.
  • Review and update medical histories.
  • Develop individualized care plans.
  • Provide education on chronic disease management.
  • Monitor patient progress and adjust care plans.
  • Complete Medicare Annual Wellness Visits via telehealth.
  • Document accurately in EHR and care-management systems.
  • Advocate for patients to access resources.
  • Participate in ongoing education and training.
  • Collaborate in AI-driven outreach programs.

Requirements

  • Licensed Registered Nurse in Georgia.
  • 3+ years of RN Case Management experience.
  • Bachelor of Science in Nursing preferred.
  • Certification in Case Management preferred.
  • Strong problem-solving skills.
  • Exceptional communication skills.
  • Ability to manage multiple priorities.
  • Proficient in technology and EHR systems.
  • Maintain patient confidentiality.
  • Compliance with company policies.
