Overview

As a Remote RN Case Manager at Guidehealth, you will support patients by helping them navigate complex health needs and improve their overall well-being from the comfort of your home.

In Short

  • Conduct telephonic assessments to understand patients' needs.
  • Develop individualized care plans for patients.

  • Participate in virtual meetings and ongoing education.
  • Advocate for patients to access necessary resources.
  • Ensure patient privacy in a secure home workspace.

Requirements

  • Active Compact Registered Nurse license in good standing.
  • 3+ years of RN Case Management experience.
  • Strong critical-thinking and problem-solving skills.
  • Ability to work independently and manage multiple priorities.
  • Outstanding communication skills.
  • High comfort level with technology and EMRs.
  • Understanding of accreditation and quality standards.
  • A quiet, private home office suitable for patient calls.
  • Alignment with Guidehealth policies and clinical protocols.
