RN Case Manager - Remote

Overview

As an RN Case Manager at Strive Health, you will play a critical role in transforming kidney care through comprehensive coordinated care for patients with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). This hybrid position involves both remote work and in-person patient visits.

Requirements

• Associate’s degree in nursing from an accredited institution.
• Current Registered Nurse (RN) license in the state of practice.
• 3+ years of related experience or certification.
• BLS or CPR Certification required.
• Reliable transportation for patient home visits.
• Proficient in electronic medical records (EMR).
• Strong communication and interpersonal skills.
• Ability to work in a hybrid-remote environment.
• Internet connectivity meeting specified requirements.
• Experience in nephrology or complex case management preferred.
