The RN Case Manager (RNCM) manages a panel of members with rising and high intensity needs, focusing on integrated chronic disease and behavioral health care.

Requirements

• 3+ years of experience in nursing.
• Graduate of an accredited nursing school (R.N.).
• Experience in chronic disease management and behavioral health.
• Strong communication and collaboration skills.
• Ability to conduct clinical assessments and develop care plans.
• Familiarity with electronic health records and care facilitation tools.

• Medical clearance requirements must be met.
