Overview

As a Case Manager RN at CVS Health, you will play a crucial role in transforming health care by advocating for members and addressing their complex health and social challenges.

In Short

  • Full-time telework role.
  • Work hours are Monday-Friday, 8:00am - 5:00pm EST.
  • Support Dual Eligible Special Needs Plan (DSNP) members.
  • Conduct assessments and develop care plans.
  • Collaborate with members, providers, and community organizations.

  • Apply motivational interviewing skills for member engagement.

Requirements

  • Active RN licensure in Pennsylvania or compact licensure.
  • Minimum 3+ years of clinical practical experience.
  • Minimum 2+ years in Case Management or related fields.
  • Excellent analytical and problem-solving skills.
  • Effective communication and organizational skills.
  • Proficiency in MS Office applications.
  • Bilingual in Spanish is preferred.
  • Certified Case Manager is a plus.
  • Ability to navigate multiple systems effectively.
  • Associates Degree or Nursing Diploma required; Bachelor's Degree preferred.
