RN Case Manager - Remote

Overview

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support complex patients in navigating the healthcare system, focusing on their medical, social, emotional, and financial needs.

In Short

• Full-Time RN Case Manager position
• Hybrid-Remote work with local travel required
• Monday to Friday schedule
• Coordinate care for patients with chronic kidney disease
• Identify and address social determinants of health
• Work autonomously and in collaboration with healthcare teams
• Passion for delivering care to complex patients
• Minimum 2 years RN experience required
• Current RN License and CPR/BLS certification needed
• Flexible and adaptable to changing priorities

Requirements

• Current RN License (BSN preferred)
• Minimum of 2 years RN experience with healthcare performance coaching
• Current CPR or BLS certification
• Willingness to travel within an assigned territory
• Intermediate computer skills (MS Word, Excel, Outlook)
• Home office with internet connectivity required
• Preferred: 2+ years of Case Management experience
• Spanish-speaking preferred in some markets
