Overview

As an RN Case Manager at DaVita Integrated Kidney Care, you will support complex patients in navigating healthcare systems, providing comprehensive care through medical assessments and interventions.

In Short

• Full-Time RN Case Manager position
• Hybrid remote work model with local travel
• Assist patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD)
• Coordinate care and manage complex patient needs
• Flexible Monday-Friday schedule
• Build empathetic relationships with patients and families
• Identify social determinants of health
• Conduct patient assessments and care transitions
• Minimum of 5 years RN experience required
• Current RN license in KY and IN required

Requirements

• Passion for patient care
• Current RN License in KY and IN
• Minimum of 5 years RN experience
• Current CPR or BLS certification
• Intermediate computer skills
• Willingness to travel within assigned territory
• Home office with internet connectivity required
• 2+ years of Case Management experience preferred
• 1+ years of utilization management experience preferred
