Case Manager RN - Remote

Overview

The Case Manager RN position at CVS Health is a remote role responsible for telephonically assessing and coordinating case management activities to promote member wellness.

• 100% remote position with no travel expected.
• Normal working hours are Monday to Friday, 8 hours between 7am to 5pm Arizona time.
• Responsible for assessing and coordinating case management activities.
• Utilize data-driven methods for individualized case management programs.
• Conduct comprehensive clinical assessments.
• Advocate for patients and promote quality outcomes.
• Provide high-level customer service.
• Utilize motivational interviewing skills for member engagement.
• 3+ years of clinical practice experience required.
• Active RN licensure in state of residence is mandatory.

Requirements

• Must have active RN licensure.
• Must work Monday through Friday, 8-hour shifts.
• Must reside in PST, MST, or Arizona Time zones.
• 3+ years of clinical practice experience required.
• 1+ years of experience with MS Office suites preferred.
• Case management experience preferred.
• Associate's degree required; BSN preferred.
