Overview

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support some of our most complex patients, assisting them in navigating a challenging healthcare system.

In Short

• Support complex patients with CKD/ESKD.
• Work autonomously and collaboratively with healthcare teams.
• Conduct medical record reviews and consultations.
• Identify and address patients' medical, social, emotional, and financial needs.
• Coordinate care and facilitate transitions of care.
• Flexible Monday to Friday schedule.

• Requires travel within assigned territory.

• Ideal candidate should live in Central Orlando area.

Requirements

• Current RN License required; BSN preferred.
• Minimum of 5 years RN experience required.
• Current CPR or BLS certification.
• Intermediate computer skills required.
• Home office with internet connectivity required.
• 2+ years of Case Management experience preferred.
• Spanish-speaking preferred.
