RN Case Manager - Hybrid/Remote

Overview

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support complex patients in navigating the healthcare system, working both remotely and in-person to provide quality care.

In Short

• Support patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD).
• Work autonomously and in collaboration with healthcare teams.
• Conduct medical record reviews and consultations.
• Identify and address social determinants of health.
• Coordinate care transitions and management of complex patients.
• Must live locally and be flexible with scheduling.
• Full-time position with a Monday to Friday schedule.
• Requires current RN License in Maryland.
• Intermediate computer skills required.
• Spanish-speaking preferred in some markets.

Requirements

• Minimum of 5 years RN experience required.
• Current CPR or BLS certification required.
• Driver’s license and willingness to travel required.
• BSN preferred.
• Experience in case management or chronic care management is a plus.
• Ability to build empathetic relationships with patients.
• Must have home office with internet connectivity.
• Flexibility in prioritizing tasks is essential.
