RN Case Manager - Hybrid/Remote

Overview

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support complex patients in navigating the healthcare system, identifying their needs, and implementing appropriate interventions.

In Short

• Support patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD).
• Work remotely with some travel to clinics.
• Full-time position with a flexible schedule.
• Coordinate care and manage complex patient needs.
• Identify social determinants of health impacting patients.
• Assess patients and address unreported medical conditions.
• Collaborate with a healthcare team for quality care.
• Minimum of 5 years RN experience required.

Requirements

• Current RN License required.
• Med-Surg experience highly preferred.
• Ability to work autonomously and collaboratively.
• Intermediate computer skills required.
• Home office with internet connectivity required.

• Strong communication and interpersonal skills.

• Experience with chronic illness management preferred.
