Requirements

• Current RN License
• BSN preferred
• 2+ years RN experience
• CPR/BLS certification
• Driver's license and willingness to travel
• Proficient in MS Word, Excel & Outlook
• Home office with internet connectivity required
• Spanish-speaking preferred in some markets
• Experience in Case Management or Chronic Care Management is a plus
• Ability to address social determinants of health
