Overview

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support complex patients in navigating the healthcare system and implement appropriate interventions based on their medical, social, emotional, and financial needs.

Requirements

• Current RN License.
• 2 years RN experience with healthcare performance coaching.
• Current CPR or BLS certification.
• Willingness to travel within an assigned territory.
• Home office with internet connectivity required.
• Passion for delivering care to patients with chronic illnesses.
• Ability to quickly adapt to changing priorities.
• Experience in Case Management or Chronic Care Management preferred.
• Ability to assess patients for unreported medical conditions.
