Overview

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support complex patients in navigating the healthcare system, working autonomously and collaboratively with the healthcare team to provide quality care.

In Short

• Full-time RN Case Manager position with a hybrid-remote schedule.
• Support patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD).
• Conduct medical record reviews and identify patient needs.
• Collaborate with healthcare teams for quality care coordination.

• Travel required for in-person visits within assigned geography.
• Minimum 2 years of RN experience required.
• Current RN License and CPR/BLS certification are mandatory.

Requirements

• Passion for delivering care to patients with complex illnesses.
• Must be flexible and willing to adapt to changing priorities.
• Current RN License required; BSN preferred.
• 2+ years of Case Management or Chronic Care Management experience preferred.
• 1+ years of utilization management experience preferred.
• Spanish-speaking preferred in some markets.
• Current driver's license required.
• Home office with internet connectivity required.
• Ability to build empathetic relationships with patients.
• Knowledge of social determinants of health.
