In Short

• Seeking a Clinical Lead Care Manager (CLCM) to serve as the client’s primary point of contact.
• Manage client cases and coordinate health care benefits.
• Collaborate with caregivers and care teams to promote wellness and recovery.
• Hybrid position requiring travel throughout Los Angeles County.
• Assess member needs in various health areas.
• Advocate on behalf of the client with healthcare professionals.
• Utilize evidence-based practices in client interactions.
• Maintain up-to-date patient health records.
• Complete all required documentation and reporting.
• Attend training as assigned.

Requirements

• MSW degree or ACSW, LCSW, or LMFT license in California required.
• 1+ year of experience as a care manager or community health worker.
• Willing to work Monday-Friday with potential evening and weekend hours.
• Knowledge of government and community resources.
• Excellent communication skills.
• Positive interpersonal skills required.
• Clean driving record and reliable transportation.
• General computer skills, knowledge of Google Workspace and MS Office.
• Bilingual (English/Spanish) is a plus.
