• Seeking a Clinical Lead Care Manager (CLCM) for Riverside County, CA.
• Acts as the primary contact for clients and coordinates with their healthcare providers.
• Manages client cases and facilitates access to care.
• Hybrid position requiring travel throughout Riverside County.
• Conducts assessments in various health areas and develops care plans.
• Advocates for clients with healthcare professionals.
• Utilizes evidence-based practices and conducts outreach activities.
• Completes documentation and maintains health records.
• Attends training as assigned.
• Must have a Master's degree in Social Work or a valid LMFT license.

Requirements

• 1+ year of experience as a care manager or community health worker.
• Working knowledge of social determinants of health.
• Excellent communication and interpersonal skills.
• Valid driver's license and reliable transportation.
• General computer skills and knowledge of Google Workspace and MS Office.
• Bilingual (English/Spanish) is a plus.
