RN Clinical Care Manager/Case Manager (Hybrid) - Remote

Overview

The Registered Nurse, Clinical Care Manager is responsible for developing and maintaining long-term relationships with physicians, physician office staff, and Healthmap members that are engaged in Healthmap’s Kidney Management Program through coordinating performance improvement activities and care management to improve health outcomes.

In Short

• Act as a liaison between Healthmap, provider practices and Healthmap members.
• Develop new provider/partner business relationships.
• Identify opportunities to improve health outcomes for Healthmap Solutions members.
• Incorporate education on Best Practice sharing and process improvement.
• Educate provider offices on Chronic Kidney Disease and related topics.
• Partner with physicians to identify members that would benefit from support.
• Educate Healthmap members on kidney health and related conditions.
• Serve as member advocate utilizing community resources.
• Accountable for individual and departmental metrics.
• Ensure timely delivery of reports to stakeholders.

Requirements

• Bachelor's degree required.
• Active, unrestricted RN license required.
• Basic Life Support (BLS) certification required.
• 3+ years of experience in healthcare services or clinical operations.
• Experience in care gap closure or care coordination preferred.
• Prior experience building relationships with healthcare providers preferred.
• Proof of valid driver's license required.
• Must reside in one of the assigned states.
