RN Clinical Care Manager/Case Manager - Hybrid - Remote

Overview

The Registered Nurse, Clinical Care Manager/Case Manager is responsible for developing and maintaining long-term relationships with physicians, physician office staff, and Healthmap members that are engaged in Healthmap’s Kidney Management Program through coordinating performance improvement activities and care management to improve health outcomes.

In Short

• Act as a liaison between Healthmap, provider practices, and Healthmap members.
• Develop new provider/partner business relationships.
• Identify opportunities to improve health outcomes.
• Incorporate education on Best Practice sharing and process improvement.
• Educate provider offices on Chronic Kidney Disease and related topics.
• Partner with physicians to identify members needing Care Navigation support.
• Educate Healthmap members on kidney health and co-morbid conditions.
• Serve as Healthmap member advocate and utilize community resources.
• Accountable for individual and departmental metrics.
• Ensure timely delivery of reports to stakeholders.

Requirements

• Bachelor's degree required.
• Active, unrestricted RN license required.
• Basic Life Support (BLS) certification required.
• 3+ years of progressive experience in healthcare services.
• 3 years of experience in care gap closure or coordination activities preferred.
• Prior experience building relationships with healthcare providers preferred.
• Proof of valid and unrestricted driver’s license required.
• Must reside in one of the assigned states.
