RN Care Navigator - Remote

Overview

The Care Navigator will be responsible for case management specific to kidney health management, facilitating high quality and cost-effective outcomes for patients.

In Short

• Handle in and outbound calls delivering world-class service to our members
• Educate kidney health and related co-morbid conditions
• Engage members into HealthMap’s Kidney Health Program
• Follow up with members based on complexity
• Serve as patient advocate to resolve concerns
• Utilize community resources in care planning
• Coordinate care with interdisciplinary care team
• Notify providers of patient needs
• Comply with HIPAA and operational policies
• Navigate technical applications like Excel and Outlook

Requirements

• Active, unrestricted RN license required
• Bachelor's degree required
• CCM preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
• Experience with Medicare and Medicaid preferred
