Overview

The Care Navigator is a remote, member-facing role centered on providing personalized guidance and support to members and their family as they navigate their behavioral health resources. In this role, the Care Navigator serves as a knowledgeable, compassionate guide - helping members understand their behavioral health options, identify barriers to care, and clarify their personal goals.

• Maintain professional communication with members and teams.
• Contribute to quarterly projects and department goals.

Requirements

• Bachelor's degree in social work, healthcare administration, nursing, or a related field.
• 2–3 years of experience in care coordination or case management.
• Proficiency in Motivational Interviewing.
• Knowledge of healthcare systems and community resources.
• Strong communication and organizational skills.
• Empathy and cultural sensitivity with diverse populations.
• Familiarity with HIPAA regulations.
• Proficiency in electronic health records and Microsoft Office Suite.
