Overview

The Care Navigator plays a vital role in ensuring that patients receive comprehensive care by connecting them with relevant community services and assisting in closing care gaps.

In Short

• Provide direct individual care navigation services to patients.
• Conduct assessments to identify factors impacting health.
• Advocate for patients in accessing community resources.
• Maintain an up-to-date database of community resources.
• Educate patients on available community resources.
• Build trust with patients and caregivers.
• Facilitate communication between healthcare providers and organizations.
• Demonstrate cultural competence when working with diverse patients.
• Maintain accurate records of patient interactions.
• Other duties as assigned, including scheduling patients.

Requirements

• Fluency in Spanish preferred.
• 3+ years of experience in care navigation or social work.
• Start-up experience a plus.
• Excellent communication skills.
• Proven skill in managing multiple communication channels.
